Option I : From LINHS Office in person on payment of Rs. 500/- (Rupees Five Hundred Only) in cash. Duly filled in form shall be submitted in person or by post along with self-attested copies of required certificate/document.

Option II : The applicants desirous of having the Application form along with Prospectus sent to them by post should remit Rs. 650/- (Rupees Six Hundred and Fifty Only) by way of Demand Draft/Money Order/Bank Transfer with due intimation to the LINHS office. DD/MO should be drawn in favour of 'LookeastEduventures Foundation' payable at Nalbari. Duly filled in form shall be submitted in person or by post along with self-attested copies of required certificated / document.

Correctly filled up Application form, together with the documents mentioned below, should be sent by Registered Post so as to reach the “THE CEO, LOOKEAST INSTITUTE OF NURSING AND HEALTH SCIENCES, Vill: Porakuchi, PO Porakuchi,Nalbari District–781337, Assam” on or before 30th November, 2025. Late, incomplete and defective applications in any respect shall be rejected.

Note :

  • The Application Form, should be filled in by the candidates legibly in their own handwriting.
  • The duly filled in Application Form shall also be signed by the parents/guardian/sponsor of the candidate.
  • No alterations(s) will be allowed in the Application Form after its submission.
  • Submission of any false or incorrect information and/or concealment of any relevant information shall result in cancellation of admission and forfeiture of any fees already paid.

    • Documents to be submitted :
    • High School (H.S.L.C.) Certificate.
    • Senior Secondary School Certificate (10+2) or Intermediate Board Certificate.
    • Marks Sheet of High School (H.S.L.C.).
    • Marks Sheet of Senior Secondary School Certificate (10+2) or Intermediate Board.
    • Qualification/Mark Sheet of Assam GNM Entrance examination.
    • Aadhar Card/PAN Card/Bank Details.
    • Two passport size photographs.
    • PRC (Optional).

      • Commencement of the Course :
      The course will commence from the 15th January or whenever the Government instruction allows.
